What information do I, your Psychotherapist, hold about you?

 

The information you have given me at Registration, which may include:

 

1. your name, phone number, email address, home address.
2. Your date of birth.
3. Details of your GP. 
4. Current medication.
5. Emergency contact name and phone number.

 

I may also keep handwritten records detailing your attendance at sessions and significant events that may occur during our work.

 

Copies of correspondence including those with third parties.

 

Invoicing information.

 

What is the lawful purpose for my keeping this information?

 

The lawful purpose is that of ‘legitimate interest’, which means that I keep information that you might reasonably expect me to keep in order to provide you with ongoing psychotherapy.

 

What will I do with the information you have given me?

 

I use it to contact you and to keep a record of the work we do together to ensure proper clinical management.

 

I will not contact you for marketing purposes. 

 

How do I store your information?

 

Your email address, telephone number, intake information and any correspondence we exchange in the course of our work, including invoices and emails, are stored on my computer and backed up on the cloud. I have access to these files via my smartphone. These devices and accounts are password protected. 

 

All paper documents associated with our work are kept in my office in a locked cabinet when not in use. Any notes that may be kept are anonymized and kept separately from your contact information. 

What third parties might be given data which might identify you?

 

1. As stated in the framework of our work, there are some circumstances when I might need to contact other professionals about our work. I would normally seek to discuss this with you first.

 

1. I would have to share your data if ordered to do so by a court of law.

I maintain an Executor List in a password protected shared folder in cloud storage with my Clinical Executors which has your name, phone number, email address and session time so that in the event of my suddenly being unable to continue our work, for example through illness, they could contact you to let you know and explore other support options with you. 

 

1. I discuss my clinical work with a senior colleague, but you are not identified by name in this supervision. 

 

I do not share your data with anyone for marketing purposes. 

 

How long do I keep your data?

 

Your name and contact details are removed from the Executor list when our work ends.  All records and contact information relating to you will be held for a maximum of seven years after the end of our work or for as long as it is necessary to comply with legal obligations or is in a legitimate interest to do so.

 

What rights do you have concerning your data?

 

You have the right to request a copy of any data that I hold on you.  This is known as a ‘subject access request.’  More information about can be obtained from the Office of the Information Commissioner www.ico.org.uk .
